Making Your First Appointment
In order to provide the best care possible, we feel it is important to fully understand your child's health history. For this reason, it is important that we first have the opportunity to review your child's health records from their previous pediatrician/physician.
- Before you send any medical records to our office, please call us at 616-949-4465 and one of our Patient Services Specialists will register your child/ren.
- Once you are registered, download the form below and send it completed through the mail, email or fax to your previous pediatrician/physician.
- After we receive your child/ren's medical records, your new pediatrician will have two weeks to review their records.
- When the doctor is finished reviewing the records, we will contact you to schedule your child/ren's initial appointment.

Transferring OUT of our practice
If you are moving or transitioning to adult medicine and would like us to transfer your records to a new provider, please download this form: https://foresthillspediatrics.com/images/transfer_out_form.pdf to give permission to send your records to your new doctor's office. Please complete and sign the form, mail or fax (616-949-6191) to us and we will send your records to your new PCP. (Remember~if the patient is 18 years or older, he/she must complete and sign the form him/herself. )
