Insurance Policy

We participate with the following insurance companies: Aetna, Blue Cross Blue Shield, Blue Care Network, Blue Choice, Community Blue PPO, Grand Valley Health Plan, Health Plan of MI, Medicaid, Messa, Physician's Care, PPOM, Priority Health, Priority Health Medicaid, and Tricare Standard. Please bring your current insurance card to each visit so that we can bill the proper insurance company. If we do not participate with your insurer, we expect you to pay us directly and we will provide receipts for you to submit to your insurance carrier. For those patients who are personally responsible for our charges, we offer a 20% discount (this does not apply to co-payments) if your charges are paid in full at the time of service. If your account has a personal balance due, you will receive a statement after each visit and then monthly if there is a past due balance.
